Message from our Board Chair and Chief Executive Officer

Dear Community Members, Partners, and Colleagues,
Every day, the people of Thames Valley Family Health Team (TVFHT) show up with a shared purpose: providing exceptional primary health care for the individuals and families who trust us. They build relationships, solve problems, and help people live healthier lives.
That commitment carried us forward throughout the past year.
At a time when access to primary health care remains one of Ontario’s greatest challenges, TVFHT embraced new opportunities to expand access while remaining deeply committed to our patients and communities.
This report tells the story of an organization that strengthened the care it provides while broadening its reach. It is a story of continuity through transformation.
Over the past year, our teams delivered comprehensive care in clinics, virtually, and throughout the community, ensuring excellence remained at the centre of everything we do. We expanded nurse practitioner-led care, launched significant access and attachment initiatives, provided virtual urgent care for children and youth across Ontario, and laid the foundation for a new strategic plan and public identity.
To our staff, affiliated physicians, partners, board and committee members, and the communities we serve: thank you. Because of your dedication, every day we continue to strengthen care, expand possibilities, and move forward together.
Sincerely,
Jodi Younger
