
Message from our Board Chair and Executive Director

Dear Community Members, Partners, and Colleagues,
We are happy to share the Thames Valley Family Health Team’s 2024–2025 Annual Report. This report looks back on a year of big changes, strong teamwork, and important progress.
Over the past year, our organization went through many transitions. We welcomed new Board members, improved how we work together, and extended our Strategic Plan to 2026. Our leadership team also changed roles for part of the year. Mike McMahon, our Executive Director, took on a temporary role with the Association of Family Health Teams Ontario (AFHTO). Jill Strong, our Director of Operations, stepped in as Interim Executive Director for the 5-month period. This smooth transition showed the strength of our leadership and our ability to support change across the health system.
Our main goal is to provide equitable access to team-based primary care that meets the needs of our communities.
Last year, we launched two new care teams. One team in Elgin County helping people who do not have a primary care provider. The other team in London focused on helping people who are homeless or at risk of homelessness. Together, these teams served more than 8,700 people, many of whom had trouble getting primary health care before.
Inside our organization, we worked hard to support our staff. We reviewed wages, made pay adjustments, and advocated for better funding across Ontario. We met with local Members of Provincial Parliament (MPPs), shared our concerns, and saw strong support from government leaders.
We looked closely at our workplace culture this past year. Our Staff Wellness and Health Equity committees led projects that helped staff feel more connected, appreciated, and supported. We also continued to track our performance to make sure we are meeting our goals and improving care.
Looking ahead, we are focused on our key priorities: building a culture of community with our staff team, growing nurse practitioner-led care, improving health equity, building leadership, and creating stronger care partnerships. The Ontario Government has announced major new funding for primary care, and we are ready to step up and lead.
To our staff, partners, patients, and Board members: thank you. Your hard work, ideas, and excellent care make everything we do possible. Together, we are building a healthier future for everyone.
