Our Partners
Working together with other organizations allows TVFHT to extend its reach to more people and provide better care. Last year, TVFHT built strong partnerships that made a big difference in several communities.
In Elgin County, TVFHT opened the Elgin Community Health Hub. This new clinic is part of a team effort with other local health organizations. It helps people who do not have a primary care provider, like a family doctor, get the care they need.
In London, a new care team focused on helping people who are homeless or at risk of homelessness was launched. This team offers medical care at 602 Queens Avenue, and visits shelters and drop-in centres to provide care where people feel safe and supported.
These projects show the power of teamwork. By working with other health organizations, community groups, and government partners, TVFHT has been able to reach people who often face barriers to care.
Moving into the next year, TVFHT is committed to continue to build these partnerships as well as finding new ways to work together. The goal is to make care easier to access, more inclusive, and better connected to the needs of the communities.

Bridging the Gap: Expanding Primary Care Access in Elgin County
Over the year, TVFHT worked together with the East Elgin Family Health Team, Central Community Health Centre, West Elgin Community Health Centre, and the Elgin Ontario Health Team to give people in St. Thomas and nearby communities better access to health care.
