Our Patients
The patients are the reason for this important work in building a healthier future, together. Last year, Thames Valley Family Health Team (TVFHT) focused on improving how they care for people and how they measure success.
- Tracked goals with tools like the Balanced Scorecard. This measures how many patients were followed up with after hospital visits, how quickly people got care, how many joined group programs, and more. These numbers help TVFHT to understand what is working and where they can improve.
- Grow group programming. New workshops were added, like Intuitive Eating and Building Foundations, which help people manage stress, trauma, and their health. More programs were made available to watch by video, so people could get the information they need on their own schedule.
- Increase self-management workshops. South West Self-Management continued to offer even more workshops across the region. These programs helped almost 1,000 people learn how to manage chronic conditions and improve their health.
- Launched a new partnership with Noojimo Health. This is a virtual mental health clinic for Indigenous patients. It gives people access to care that respects their culture and traditions.
The goal is to make sure every patient feels supported, understood, and cared for. TVFHT is working to make care better for everyone, with better data, new programs, and stronger partnerships.
