Message from our Board Chair and Executive Director

We are excited to share our 2024 Annual Report! This report summarizes the achievements, milestones, and continuous efforts undertaken over the past few years to enhance the quality of care we provide to our patients.

We would like to thank our team of dedicated health and administrative professionals who are committed to delivering excellent, team-based primary care in a collaborative model aimed at addressing the diverse health needs of our community. Thank you also to our physician partners, whose expertise and collaboration are vital in providing the highest standard of care to our patients.

We believe that integrated care is essential to achieving the best health outcomes, and our approach ensures that each patient receives personalized, coordinated, and continuous care. This report provides a detailed overview of our performance metrics, patient outcomes, and the initiatives that have driven our success. It also highlights the stories and experiences of our patients and team members, illustrating the real-world impact of our work.

In 2022, we began planning for our shift from a site-based model to a centralized model — an Integrated Team Care Network — based in three or four anchor locations versus the 19 locations we currently occupy. This will allow us to maximize our resources and enable us to eventually extend team-based care into our communities. This report will provide more details on our core service design and how we are starting to implement this change.

As we reflect on the past year, we are proud of the progress we have made and are inspired by the challenges that lie ahead. Our commitment to delivering high-quality, team-based primary care remains steadfast, and we look forward to continuing our journey towards better health for all.
