Emphasizing the importance of understanding and addressing the individual needs of our patients, we are enhancing patient care through a series of strategic initiatives aimed at improving patient satisfaction and engagement, broadening access and improving outcomes. These initiatives are grounded in health equity and cultural safety principles.
Our efforts include:
- Expanding access to our team of Interdisciplinary Health Professionals by offering free groups to the public that focus on healthy changes that can influence things like weight, sleep, and overall quality of life.
- Redesigning our core services towards an Integrated Team Care Network to support patients with complex health needs.
- Providing translation services through Voyce to ensure patients receive equitable care.
- Providing transportation support via bus tickets and cab rides to improve accessibility for in-person appointments.
Future initiatives include launching primary care services at 602 Queens Avenue for those without access to a family physician, particularly individuals experiencing homelessness, and expanding team-based care in Elgin County for vulnerable populations without a primary care provider.
