TVFHT Staff to share innovative projects and research at AFHTO Conference

We are delighted to announce that TVFHT staff will be highlighted at this year’s AFHTO conference.

The AFHTO “Power of Primary Care” Conference will take place on October 23-24, 2025, in Toronto. This conference focuses on innovation and collaboration in primary care. AFHTO received over 125 abstracts and seeing THFHT so well represented is an incredible achievement.

Lisa Vreugdenhil, Director, Clinical Services will present a poster called Attachment from Health Care Connect List:  Falling through the Cracks. It will explain what to expect when connecting patients from the Health Care Connect (HCC) list and describing the gaps. Data will be shared about outcomes of attempted attachment to primary care from the HCC list.

Jillian Leslie, Quality Improvement – Data Analyst  and Katie McDonald, Medical Directives Coordinator & Clinical Support will jointly present Beyond the Numbers: Reimagining Hospital Reporting Management to Improve Discharge Follow-Up. This presentation explores a Hospital Reporting Management initiative that re-examines hospital discharge and follow-up practices. Prompted by concerning gaps in follow-up care, our team investigated what was happening post-discharge—and more importantly, what wasn’t.

 Elizabeth Smith, Interprofessional Clinical Lead, and Lisa Vreugdenhil, Director, Clinical Services will present: Innovation in Access: Integrating Nurse Practitioners as Most Responsible Providers in High-Performing Primary Care Teams. This plenary session will explore how  Minto-Mapleton Family Health Team and the Thames Valley Family Health Team are leading the way in reimagining primary care delivery and leadership by through the NP-MRP model.

Payal Patel, Interprofessional Clinical Lead, will be presenting on Interprofessional Educators: The Missing Link in Primary Care Training. This session will  outline the benefits of HPEs-FM models of collaboration. It will identify practical strategies and teaching tools.

Patel will also be presenting on Bridging Knowledge and Practice:  Family Health Team Pharmacists Advancing Care through Academic Detailing. This session will help atendees understand how an Academic Detailing Service was implemented across 4 FHTs. It will explore practical approaches for FHT adoptions.

Katelyn Thompson, Registered Nurse, will be representing our Health & Homelessness Interprofessional Primary Care Team with a poster display entitled: Equity In Action: Expanding Access to Primary Care for Homeless and Marginalized Populations. This project highlights a critical gap in understanding how individuals experiencing homelessness in London, Ontario, face significant barriers to primary health care. They will share local data and explore the systemic challenges that disproportionately affect this population.

 Amanda Walker, Mental Health Counselor, will be representing our Collaborative Care Network Project Team with a poster display entitled: From Innovation to Implementation: Scaling Collaborative Mental Health Care in Primary Care Using a Co-Design and Quality Improvement Toolkit.

This project works on practical implementation of a Collaborative Care Model for mental health and addictions in primary care, using a structured, evidence-informed, and co-designed approach. Drawing on the Thames Valley Mental Health and Addictions Collaborative Care Network experience, they will explore how three diverse primary care clinics (academic, rural, urban) co-designed and implemented tailored models of collaborative care grounded in population health management and measurement-based care.
