Healthcare partners across London and Middlesex are marking an important milestone in improving access to primary care. As of September 30, 12,489 more people in London and Middlesex have been connected to a family doctor, nurse practitioner, and/or primary care team.
This achievement reflects the collaborative work of the Middlesex London Ontario Health Team (MLOHT), Middlesex London Primary Care Network (MLPCN), Thames Valley Family Health Team (TVFHT), Interprofessional Primary Care Team partners, the Ministry of Health, Health Care Connect, family physicians, nurse practitioners, and healthcare organizations throughout the community.
The milestone ties to the Province of Ontario’s recent announcement that more than 511,000 additional Ontarians have been connected to a family doctor or primary care team. Locally, it demonstrates how partnership-based solutions can improve access to care while strengthening the primary care system.
“Improving access to primary care requires coordinated action across the healthcare system,” said Jill Strong, Executive Director – People & Performance, TVFHT. “This milestone reflects the commitment of many organizations and hundreds of healthcare professionals and support staff who have worked together to connect more people to the essential care they need.”
The initiative was supported through provincial investments in primary care expansion and local attachment efforts. As part of the province’s Primary Care Action Plan, the region received $5.3M in funding in August 2025 to expand access to team-based primary care in Middlesex London. Partner organizations worked together to recruit healthcare professionals, establish new models of care, expand clinical capacity, and improve patient pathways.
“Connecting over 12,000 people to primary care in our community demonstrates what is possible when local partners work together,” says Amber Alpaugh-Bishop, MLOHT Executive Director “The impact of these connections are significant, with better health outcomes for local residents and a better functioning health system.”
For residents, the impact extends beyond the numbers.
Access to primary care improves opportunities for preventive care, chronic disease management, health promotion, mental health support, and coordination across the healthcare system. It creates a consistent point of access for care closer to home.
Attachment Efforts Continue
While this milestone represents significant progress, healthcare leaders acknowledge that the need remains substantial. Recent Ministry of Health data indicates that approximately 87,000 people in Middlesex London are still unattached to primary care.
“We are proud of what has been accomplished through this initiative to date, but our work is not finished,” said Strong. “Every new attachment improves health outcomes for individuals and families. At the same time, we know there are thousands of residents who are still looking for a primary care provider. We will continue to build on this momentum and advance the work of expanding access across our region.”
In April 2026, Middlesex London received additional provincial funding through to further expand Primary Care Action Team Phase 2 (PCAT2) primary care capacity and support additional attachments across the region.
