The Health & Homelessness team gives this underserved group access to wraparound care.
They offer a range of services on-site at 602 Queens Avenue as well as in various locations in the community.
Services provided include
- medical care for acute and chronic illnesses
- preventative health screening
- immunizations & vaccines
- foot and wound care
- medication and injection help
- diagnostic test orders
- lab collection (blood & urine)
- women’s health and family planning
- system navigation (such as connecting patients to other services-dental, optometry, etc., accompanied travel to medical appointments and help with government forms)
This team believes that care is showing up where people are. They deliver low-barrier, trauma-informed care in accessible spaces. They focus on voluntary attachment with respect for patient choice.
