
During Social Work Week we celebrate the dedication of our social workers, who are integral in a primary care team.
Always, but especially from March 2-8, 2026, we appreciate the contributions of our team of social workers at Thames Valley Family Health Team. This year’s theme, “Care. Insight. Together,” perfectly captures the interconnected strength of social work and highlights both the expertise and the empathy that are necessary in this line of work.
Social Workers in Primary Care
By integrating social services into primary care, Social Workers help reduce hospital visits, improve mental wellness, and empower patients. Their ability to address the root causes of health challenges leads to better patient outcomes and stronger communities.
Social workers are integral in assisting individuals to develop skills and navigate challenges such as depression, anxiety, stress, and adaptation to illness. Our team of social workers also facilitate groups, including Introduction to Mindfulness and Living Mindfully which are both open to the public.
Acting as the Bridge
Our social workers are often the bridge to care. They provide critical support, working to reduce the strain on hospitals and emergency departments.
Their work addresses the social determinants of health – such as housing, employment, and access to community resources – to help create a more holistic and effective care experience.
This Social Work Week we applaud the significant contributions of these health professionals. Their work makes a real difference in the lives of all their patients as well as supports a robust interprofessional team of clinicians.
