Celebrating Occupational Therapists: The Everyday Problem Solvers Behind Independent Living


October is Occupational Therapy month in Canada, and we want to celebrate the incredible work happening here at Thames Valley Family Health Team (TVFHT). Our Occupational Therapists (OTs) are wide-scope clinicians — they’re preventative problem solvers who help patients live safely, independently, and purposefully, despite aging or chronic health conditions.

Seeing the Whole Person

What makes an OT’s approach so valuable is that they look at the whole picture: a person’s environment, their physical and cognitive health, their goals, and — most importantly — what matters most to them. From there, TVFHT’s OTs offer education, tools, and hands-on support to help patients thrive in their daily lives.

What Makes Primary Care OT Different?

Occupational therapy looks different depending on where it’s practiced. Unlike OTs working in hospitals or rehab centres, TVFHT’s primary care OTs focus on:

  • Early intervention — preventing injury, crisis, or hospital admissions before they happen.
  • Health promotion — empowering patients to manage chronic conditions and age well.
  • A strengths-based approach — starting with what matters to the patient and building a path to get there.

By identifying challenges early, OTs help patients recognize their own strengths and use them to overcome barriers — rather than waiting for a crisis to intervene.

How TVFHT’s OTs Make a Difference

The team’s impact shows up in several key areas:

Functional Assessments & Early Interventions
OTs help patients manage the impact of health conditions on daily tasks, using their unique clinical lens to catch issues early and connect patients to timely support — whether that’s education, adaptive strategies, assistive devices, or referrals to other clinicians on the team.

Falls Prevention & Confidence Building
Through targeted education and tools, OTs help reduce fall risk while measurably boosting patients’ confidence and functional independence.

Cognitive & Driving Assessments
OTs conduct comprehensive cognitive assessments that support primary care providers in diagnosing conditions like dementia and mild cognitive impairment. They also link cognitive function to real-world performance and proactively screen for driving safety — often long before problems arise — helping patients and families plan ahead.

Caregiver & Family Support
Caring for a loved one can be exhausting. OTs help reduce caregiver burnout through practical support and education, and guide families through difficult decisions, particularly around cognitive decline or complex care needs.

Team-Based Referrals & Navigation
OTs act as a connective hub, linking patients to other professionals both within TVFHT and across the broader community — building bridges that support holistic care.

A Year of Impact

The team has plenty to be proud of from the past year:

  • Falls Prevention Groups (Steady & Strong): 100% of survey respondents would recommend the group to others and 87.5% saw improved scores on the Falls Efficacy Scale. Registration is open on our Programs page for Steady & Strong.
  • Memory Clinics: Playing an integral role in supporting early dementia diagnosis.
  • Driving Screens: Helping providers and families assess driving readiness and safety.
  • Chronic Disease & Aging Education: Supporting patients to stay independent longer.
  • Functional Pain Management: Helping people with chronic pain regain function — going beyond simply treating the pain itself.
