March is Pharmacy Appreciation Month

This March, during Pharmacy Appreciation Month, we recognize the knowledge, expertise and commitment pharmacists offer patients in our communities.

At Thames Valley Family Health Team, our pharmacists are embedded directly into primary care — working alongside physicians, nurse practitioners, and other team members to optimize medications, improve safety, and strengthen outcomes for complex patients.

From supporting those with chronic illnesses, like diabetes and heart disease, to discussing medication adjustments, their expertise makes a big difference for our patients.

The Impact of Pharmacists in Primary Care

Primary Care Pharmacists are not “add-ons” — they are essential to high-functioning team-based care.

• Comprehensive medication reviews and deprescribing

• Chronic disease optimization (diabetes, hypertension, COPD, heart failure)

• Real-time prescribing support

• EMR-based case finding and quality improvement

• Academic detailing in partnership with the Centre for Effective Practice

• Teaching Family Medicine residents and interprofessional learners

This is what team-based care looks like in practice — not parallel work, but integrated, collaborative, evidence-informed care.

Their reach extends beyond TVHFT patients. Our pharmacists participate in running several of our free group programs. These groups are on topics such as sleep improvement and smoking cessation as well as ones that focus on helping seniors live more independently.

In their work alongside physicians and other healthcare providers, pharmacists ensure patients receive safe, accessible, ethical and effective pharmacy care. They are an integral part of Thames Valley Family Health Team’s success, and we count on their guidance to help us deliver the best possible care for patients and their families.

As Ontario continues to invest in primary care transformation, the role of the primary care pharmacist is not just valuable — it is foundational.
