Celebrating Dietitians during Nutrition Month 2025: Nourish to Flourish

We are excited to celebrate the dedicated Registered Dietitians at Thames Valley Family Health Team (TVFHT) this Nutrition Month! This year’s national theme, “Nourish to Flourish,” highlights the essential role of nutrition in achieving optimal health and well-being. A healthy diet is not just about what we eat – it’s about fostering a lifestyle that supports long-term wellness.
The Role of Dietitians in Primary Care
Dietitians at TVFHT help the people we support flourish. As experts in nutrition and health, they collaborate with individuals to develop personalized plans tailored to their specific needs, whether it’s managing chronic conditions, achieving balanced nutrition, or forming sustainable, health-promoting habits. Their guidance is a cornerstone of comprehensive care, empowering patients with the knowledge and tools they need to lead healthier lives.
Dietitians are an integral part of our interdisciplinary health care team, working alongside physicians, nurse practitioners, and other allied health professionals to provide patient-centred care. Through this collaborative approach, our dietitians help address a broad range of health concerns, including heart health, diabetes management, digestive issues, and more.
Expanding Access to Nutrition Support
We recognize the importance of making nutrition services more accessible to help our whole community flourish. That’s why we offer free virtual group programs such as:
- Best Weight – A science-based approach to weight management, focusing on sustainable lifestyle changes. Now available as a virtual group and a self-guided video!
- Heart Health – A program dedicated to heart-healthy eating habits and cardiovascular wellness. Available as a virtual group and self-guided video series.
- Sweet Talk – A support program for individuals managing blood sugar levels and diabetes. Available as a virtual group.
These programs help patients build practical skills related to nutrition, physical activity, and overall health behaviors that may reduce chronic disease risk. By offering these virtual sessions, we ensure that more individuals can access expert dietary advice from the comfort of their own homes.
Meeting the Unique Needs of Primary Care
Dietitians in primary care play a crucial role in serving a diverse population with a wide range of nutritional needs. They are highly adaptable, addressing everything from pediatric nutrition to geriatric care, chronic disease prevention, and rehabilitation support. This versatility allows them to make a significant impact on patient health, promoting both prevention and recovery.
Thank You to Our Dietitians
As we celebrate Nutrition Month 2025, we extend our deepest gratitude to our dedicated dietitians. Their expertise, compassion, and commitment to patient care make a profound difference in the lives of individuals and families in our community. Thank you for all you do to nourish, educate, and empower the people we support!
